Cayuse IRB Submission

There are no longer multiple protocol application forms.  Every new study will have the same initial submission process.  The IRB office will make the determination regarding what type of review will be performed.

Cayuse IRB forms include:

  • Initial (new protocol submission)
  • Legacy/Existing Protocol (pre-existing study submission/only used for studies existing prior to May 1, 2016)
  • Modification/Amendment (amendment/modification submission, including personnel changes)
  • Continuing Review/Renewal (renewal of study)
  • Withdrawal (withdraw study submission)
  • Closure/Final Report (close study)
  • Incident/Problem Report (reporting incidents/adverse events/problems)

Please make sure to check the Clutter and Junk email folders if you are expecting an email from Cayuse but not yet received it. Cayuse IRB notifications will be sent when a new submission is sent and when any action item/task is required of the researchers.  Please pay close attention to any email from irboffice@ucmerced.edu
